What do we offer?

Are you growing older and worrying about being able to stay at home?
Are you living with a disability?
Are you caring for a family member or friend?

Our community-based services include options to ensure you are eating well, getting out and about and managing your health and well being.  We work with you and the people who support you, to find out the things that you want to achieve and that are important to you.  We then help put in place the right solutions with you.

Some of the services that are offered as part of help at home are:

  • Meals on Wheels
  • Community Transport Program
  • District Nursing
  • Social support groups
  • Carer support groups
  • Flexible Home-Based Aged Care (Sea Lake only)
  • Continence support
  • Podiatry
  • Occupational Therapy
  • Exercise and fitness groups
  • Dietetics

Community availability

These programs are delivered in a range of ways in Ouyen and Sea Lake and to outreach areas including Patchewollock, Underbool, Murrayville and Sea Lake districts.

How much will it cost?

MTHCS receive funding from a number of Government sources to provide help at home services. Each service has a different cost and discounted rate for eligible consumers. View the Community Services Fees Policy for the latest costs.

How to access this service?

You will need to be assessed by My Aged Care. For more information on how to access help at home services or for general enquiries call the Ouyen Service Centre on (03) 5092 1111 or the Sea Lake Service Centre on (03) 5070 2155.
