Position summary
The Individual Support Worker provides a valued and key role in providing care and support that enhances the
ability of and encourages consumers to actively meet their unique needs. Individual Support Workers are
compassionate, passionate and dedicated to planning and implementing a person centred care plan and
individual supports that are appropriate to the needs and preferences of the person. The program will vary from
person to person depending upon their individual support needs.
Selection criteria
Essential:
- Excellent communication and organisational skills.
- Maintain a professional relationship with clients and their carer and adhere to organisational code of conduct.
- Current Driver's Licence.
- Current Police Check.
- Proficiency with computer skills.
- Works well in a team environment, as a team member and independently.
- Recognised Community Services or Disability qualifications or willingness to work toward a Diploma in one of these areas.
Desirable:
- Previous experience working in the disability/community care sector.
- Knowledge and experience in developing and implementing activities programs to meet individual and client and carer needs.
- Genuine commitment to enhancing the quality of life of persons whom have a disability.
- Knowledge of Individual Support Package Guidelines.
- Knowledge of the Charter of Human Rights.
Key responsibilities
Professional:
- Respectfully communicate with client and their carer using a person centred approach.
- Communicates effectively with health professionals and other health care providers.
- Have empathy, patience and a genuine passion for supporting client to live their best lives. Consider the individuals abilities, needs and interests, and implement a person centred care plan and individual
supports, directed by the individual and their carer.
- Complete comprehensive assessments, documenting individual needs, goals and preferences.
- Involve the consumer and their family in developing an individualised program ensuring maximising the
individual's capacity for independence and empowerment.
- Manage resources including vehicles, identifying issues, client and non-client related and reporting them
in a timely manner per MTHCS incident reporting procedure.
- Involvement in annual care team meetings and implement care plan strategies as directed.
- Monitor and review program with the aim of continual improvement and to ensure activities meet the
needs of the client and their carer.
- Document client progress, record data and maintaining detailed record of care after each session.
- Support staff with Community programs as required.
- Assist with facility based tasks as directed such as bookings, audits ect
- Ensure appropriate, regular and timely communication with consumers and stakeholders about the
organisation and develop strategies to encourage consumer participation.
- Provide leadership and clear, effective direction to the volunteers who provide support to the organisation.
- Other duties as directed.
Organisational:
- To ensure all staff comply with regulatory requirements of relevance and MTHCS policies and procedures.
- Support systems that provide safe, high quality care that increases patient experience and patient centred
care, and active understanding of legislative compliance regarding restrictive practices.
- To comply with MTHCS Employee Code of Conduct.
- To ensure that MTHCS Infection Control guidelines are fully implemented.
- Demonstrate awareness and understanding of diversity and culture.
- MTHCS has a responsibility to maintain current policy; it is the responsibility of all staff to be aware of current policy.
- Maintains confidentiality/privacy of client information as per MTHCS policies and procedures.

- Help MTHCS meet our duty of care, such as by undertaking hazard inspections.
- Comply with all MTHCS OHS policies and procedures.
- Ensure they do not interfere with or misuse anything provided in the interests of health and safety.
Section 21 of the Occupational Health & Safety Act 2004, requires an Employer to provide and maintain, so far as is reasonable and practicable, a safe workplace for all employees and ensure that no employee is placed at risk of injury while performing
the duties and responsibilities required by their position.
To comply with this obligation, MTHCS requires all candidates/employees to have the mental and physical capacity to safely perform the inherent requirements of the position they are applying for (or have been appointed to).
If a risk is identified and fitness to safely perform the duties of the position needs to be determined, MTHCS with the agreement of the candidate/employee, will arrange an independent medical assessment to determine capacity to safely perform the inherent
requirements of the position to minimise the risk of injury or aggravation of any pre-existing condition the candidate/employee may have.
